If a carrier has lost or damaged a package please complete the form below (also available at this link). Once the information has been received we will reach out to you to confirm any needed details and gather documentation.


After the claim has been submitted to the carrier the recipient may be contacted to verify information. Please ensure the recipient is responsive to the carrier's requests or a claim may be denied. 


Claims are typically resolved within 1-2 weeks. However, delays due to COVID-19 have caused additional processing time.


Note: All FedEx and UPS packages include $100 of insurance. However, both carriers will only cover the replacement cost (not retail value) of items.
